1. Operator and acceptance

This website is operated by KP RX LLC, doing business as PROCAREPHARMACYon Hylan, located at 1300 Hylan Blvd, Staten Island, NY 10305. By using this website, you agree to these Terms of Use. If you do not agree, do not use the website.

2. Informational purpose only

Website content is general information and is not medical advice, diagnosis, treatment, prescribing, pharmacist counseling, or a substitute for professional judgment. Always consult an appropriately licensed health professional about medications and health concerns. Do not delay seeking care because of information on this website.

3. No online prescribing or internet dispensing

This website does not diagnose, prescribe, create a patient-practitioner relationship, accept prescription uploads, sell prescription drugs online, or complete prescription transactions. Prescription medications are dispensed only after receipt and review of a valid prescription and satisfaction of all legal, clinical, insurance, and pharmacy requirements.

Telephone, SMS, and email links are provided for convenience. Sending a message does not guarantee that a prescription was received, accepted, transferred, filled, or ready. You must receive confirmation from pharmacy staff.

4. OTC catalog and order requests

Catalog listings and representative store photographs are provided to help customers browse products. Product names and prices are imported from the pharmacy point-of-sale system, but exact package photos, labels, stock status, fulfillment options, and purchase restrictions may still require staff verification. UPC and NDC identifiers are kept private and used by pharmacy staff to identify requested products. An NDC is an identifier and does not by itself mean that FDA has approved a product.

Adding an item to a request list or submitting an item-finder form is an inquiry, not a completed purchase, reservation, or guarantee of price or availability. Pharmacy staff must confirm the exact item, package, quantity, price, pickup or delivery terms, and payment method. Always read and follow the complete label supplied with the product.

5. Interaction screening

The interaction tool, when enabled, screens exact medication-name matches through a licensed clinical-data provider. It is not guaranteed to identify every interaction and does not account for every dose, condition, allergy, laboratory result, food, alcohol, supplement, pregnancy, or treatment goal. A result is not medical advice, diagnosis, prescribing, or pharmacist counseling. Do not start, stop, or change a medication based only on the tool. Consult a pharmacist, prescriber, or other appropriately licensed professional.

6. Privacy and communications

General email and SMS may not be secure. Do not place medication names, prescription numbers, dates of birth, insurance details, diagnoses, payment information, or other sensitive information in product-request or item-finder forms, general email, or text. The secure prescription transfer form may be used for the transfer details it specifically requests. Do not upload a prescription, pharmacy label, or insurance card. Call the pharmacy for instructions. Carrier messaging and data charges may apply.

Website information practices are described in our Website Privacy Policy. Protected health information handled in providing pharmacy services is governed by our HIPAA Notice of Privacy Practices and applicable law.

7. Pharmacy services and availability

Services, products, hours, delivery areas, vaccine availability, discounts, insurance participation, and pricing may change and may be subject to eligibility, supply, prescriber authorization, plan rules, clinical review, and applicable law. Website content is not a guarantee of coverage, price, stock, delivery, or treatment outcome.

We provide services only in jurisdictions where we and the professionals involved are appropriately licensed or otherwise authorized. Nothing on this website is an offer to dispense or deliver prescription medication where doing so would be unlawful.

8. Compounding access

PROCAREPHARMACYon Hylan does not compound medications on site. When clinically and legally appropriate, we may help patients access patient-specific compounded preparations through appropriately licensed compounding partners after receipt of a valid prescription. Availability is not guaranteed. Compounded preparations are not reviewed or approved by FDA as finished drug products in the same manner as FDA-approved manufactured drugs.

9. Accuracy and third-party resources

We work to keep website information accurate and current but do not warrant that every item is complete, error-free, or continuously available. Links to mapping, telephone, messaging, email, government, regulatory, or other third-party services are provided for convenience. We do not control their content, availability, security, or privacy practices, and a link does not imply endorsement or affiliation.

10. Acceptable use

You may not use this website to:

• Violate any law or the rights of another person.
• Attempt unauthorized access, interfere with operation, introduce malicious code, or bypass security controls.
• Misrepresent your identity or submit false, fraudulent, or misleading information.
