1. Scope

This Website Privacy Policy applies to information collected through this public website by KP RX LLC, doing business as PROCAREPHARMACYon Hylan ("Procare," "we," "us," or "our"). It does not replace our HIPAA Notice of Privacy Practices. If information is protected health information under HIPAA or another health privacy law, the HIPAA Notice and applicable law control.

This website provides public pharmacy information, an OTC and wellness catalog, product-availability and item-finder request forms, a secure prescription transfer request form, an optional medication-interaction screening tool, and links that allow you to call, text, email, or obtain directions. The website does not currently provide patient accounts, prescription uploads, online payment processing, or a patient portal.

2. Information we may collect

Information you choose to provide

If you submit a product-availability or item-finder request, we collect the name, email address, optional telephone number, product selections, description, message, and optional product photo that you provide. If you contact us by phone, text, or email, we receive the contact information and message content you choose to provide.

Prescription transfer requests

If you use the secure prescription transfer form, we collect the patient name, date of birth, address, medications to transfer, current pharmacy name, and current pharmacy telephone number. The details are encrypted and stored for authorized pharmacy staff to review in a restricted administrative portal. The notification email sent to the pharmacy contains a request number and sign-in link, but no patient or prescription details.

The medication selector uses the U.S. National Library of Medicine's public RxNorm service to suggest current medication names, strengths, and dosage forms. Only the medication search term is sent to that service; the patient's name, date of birth, address, pharmacy information, and completed transfer request are not sent with it. Patient addresses are not submitted to Google Maps or another mapping service.

Technical information

Our hosting and security providers may automatically process limited technical information needed to deliver and protect the website, such as IP address, browser type, device type, requested pages, timestamps, and security or error logs.

OTC catalog and order-list information

Product searches and the items you add to a request list may be stored locally in your browser so the list can remain available on your device. We receive the selected products only when you submit the form. Private inventory identifiers associated with those products are added on our server to the email delivered to pharmacy staff; they are not displayed in the public catalog.

Photos and file metadata

An optional item-finder photo is attached to the request email and may contain information embedded by your camera or device. Do not upload a prescription, pharmacy label, insurance card, government identification, or an image containing protected health information. We do not use the item-finder form for prescription services.

Interaction screening

If you use the interaction-screening tool, the medication names you enter are sent to public U.S. Food and Drug Administration drug-label services and U.S. National Library of Medicine medication-class services to identify label-based interaction information. Do not enter identifying information. Procare does not use the tool to create a patient profile, and the website does not retain the medication names in its activity measurements. These public services and the hosting provider may process limited technical logs under their own terms and security practices.

Private website measurement

We keep first-party counts of clicks to call, text, or obtain directions; product-detail views; completed cart, item-finder, and prescription-transfer requests; completed interaction checks; and interaction-tool failures. These measurements are available only in the pharmacy's restricted administrative dashboard. We do not add names, email addresses, telephone numbers, street addresses, medication names, form contents, IP addresses, browser identifiers, or advertising identifiers to these measurement records.

Cookies and tracking

We do not currently use third-party advertising pixels or behavioral advertising trackers on this website. Our hosting platform may use strictly necessary technologies to provide security, routing, and basic site functionality. If our practices materially change, we will update this policy and provide any notice or choices required by law.

3. How we use information

• To operate, secure, troubleshoot, and improve the website.

• To provide product information and respond to OTC product, price, availability, pickup, or delivery inquiries.
• To attach optional product photos to item-finder emails requested by the user.

4. How information may be disclosed

We may disclose website information to service providers that host, secure, maintain, or support the website; deliver request emails; provide licensed clinical interaction data; to professional advisers; when required by law or a lawful government request; or when reasonably necessary to protect rights, safety, and security. Service providers are permitted to use information only for the services they provide to us and subject to applicable contractual and legal requirements.

We do not sell personal information obtained through this website. We do not use website information for cross-context behavioral advertising.

5. Security and retention

We use reasonable administrative, technical, and physical safeguards appropriate to the information we maintain. No website, email, SMS transmission, or storage system can be guaranteed completely secure. Prescription transfer details are encrypted at rest and made available only through the restricted pharmacy administrative portal; alert emails exclude those details. Product and item-finder submissions are delivered to the pharmacy by email and may be retained in the pharmacy mailbox. We retain information only as long as reasonably necessary for the purposes described here, to meet legal and regulatory obligations, to resolve disputes, and to enforce agreements.

6. Your choices and requests

You may ask about personal information you submitted directly to us and request correction or deletion, subject to identity verification and any legal, pharmacy-record, insurance, or retention requirements. Privacy choices that apply to protected health information are described in our HIPAA Notice.

Because we do not use behavioral advertising trackers, the website does not currently respond differently to browser "Do Not Track" signals. We will honor legally required browser-based privacy signals if they become applicable to our practices.

7. Children and external links

This general-audience website is not directed to children under 13 and is not designed to collect information directly from children. The website may contain links to outside services, such as mapping, telephone, messaging, email, government, or regulatory websites. Their privacy practices are governed by their own policies.

8. Changes to this policy

We may update this policy as our website or legal obligations change. The effective date above identifies the current version. Material changes will be posted prominently when required.
