Protect medical information
Do not include medication names, prescription numbers, dates of birth, insurance information, or diagnoses in general email. Call the pharmacy for a request involving pharmacy records or protected health information.

Verification and response
We may verify your identity and authority before disclosing, changing, or deleting information. We will respond as required by applicable law. Some information may be retained when required for pharmacy, health-care, legal, security, fraud-prevention, or recordkeeping purposes.
HIPAA requests
Rights involving protected health information are explained in the HIPAA Notice of Privacy Practices. Pharmacy staff can provide the appropriate process by telephone or in person.
